Estou fazendo um sistema de UpLoad de arquivos no meu site, e quando eu termino de preencher o formulario, o arquivo esta até indo para a pasta que selecionei para fazer o upload porem depois que envia o arquivo ele trava em uma pagina com os seguintes erros:

ACHEI UM ARQUIVO QUE INCLUI ALGUNS ARQUIVOS NO INCLUDES DAI RESOLVEU...
